The District would like to thank you for your continued support in the health offices. As you and your child plan for the upcoming school year, please keep the following in mind:

Health Appraisals (COMPLETED BY PARENT / GUARDIAN)

A physical exam should be performed by your child’s NYS licensed Health Care Provider. It is required for all students entering the school district for the first time and students in Pre-Kindergarten or Kindergarten, Grades 1, 3,5, 7, 9, and 11. A copy must be given to the school. If your child has an appointment for a physical exam after this date, please notify the Health Office.

Immunization Requirements

  • NY State law requires all students to be up to date on their immunizations in order to attend school. Please contact your child’s doctor to ensure your child has the required vaccinations. See the Vaccination Requirements on the school’s website.
  • Students entering 6th grade must receive a (Tdap) vaccine by their 11th birthday.
  • Students entering 7th grade must receive their first Meningococcal conjugate vaccine.
  • Students entering 12th grade must receive their second Meningococcal conjugate vaccine.


***If your child is not up to date on their required vaccines per NYS guidelines, THEY WILL NOT BE ALLOWED TO ATTEND SCHOOL UNTIL THE REQUIRED IMMUNIZATIONS ARE OBTAINED. This is an NYS requirement.***

Vision/Hearing/Scoliosis

All students entering UPK, K, Grades 1,3,5,7, and 11 will have their hearing and vision checked at school, If not checked by their healthcare provider at their routine health appraisal. Girls in grades 5 & 7 and boys in grade 9 will be checked for scoliosis at school. You will be alerted if there are any abnormal results. If you would rather have this screening done by your student’s healthcare provider, please let your child’s health office know. A copy of their results must be provided to the health office.

Medication

If your child is required to take any medications during school hours (whether a daily or an “as needed” medication this includes all prescription and over-the-counter medications), we will need a written order from the doctor, also signed by you, the parent/guardian. If your child is able to self-carry/self-administer a medication and that medication is for treating a potential life-threatening condition (diabetes, asthma, severe allergy, or seizures), we will need a signed order from the doctor that indicates they may self-carry the medication. Elementary students are NOT allowed to self-carry/self-administer medications.

All medication must be delivered to the nurse's office by a parent/guardian. Students are not permitted to have ANY medication in their possession (unless they have been given written medical approval). This includes prescription and “over-the-counter” medications (such as Advil and Tylenol).

If your child has Asthma, Anaphylaxis due to a Bee Sting or Food Allergy, Seizures, Latex Allergy, Hypoglycemia, Hyperglycemia, or any other life-threatening emergencies, they require an Emergency Care Plan (ECP). Your child’s doctor will be able to provide the ECP, there are also example ECP forms on the school website. It will be shared with trained lay staff, including bus drivers and monitors, who supervise your child when a nurse may not be immediately available. You and your child’s doctor must both sign and return the final version to the nurse’s office.. All ECP’s need to be renewed yearly.

Middle/High School Sports

Fall Sports are slated to begin on the following dates: August 19th (Varsity Football), August 21st (all other JV/Varsity Sports), August 24th (Modified Football), and August 28th (all other Modified Sports). You will need to sign your child up on Family ID www.familyid.com in order for them to participate. Registration for fall sports will open approximately one month prior to the start of the season. Your child will need to have a physical exam by a licensed healthcare provider dated within the past 12 months on file prior to the start of sports. All athletes also need to have a current Impact test on file. After signing your child up on Family ID, if they need an updated physical or Impact test, you will be contacted by the Health Office.

Additional Information

Please be aware that your healthcare provider cannot share your child’s health information with our school’s health staff without your signed permission. Please talk to your provider about signing their consent form for the school at the time of your child’s appointment for the examination. Forms can be found below:

We suggest you make copies of the completed forms for your own records before sending them to the school health office. All Forms may be faxed to Elementary Health Office at (585) 289-2114 or the Middle/High School Health Office at(585) 289-2112 or scanned and emailed to huber@redjacket.org or brown@redjacket.org. Please feel free to call the health office with any questions or concerns.

District Wellness Policy

The District is committed to providing a school environment that promotes and protects children's health, well-being, and the ability to learn by fostering healthy eating and physical activity. The District has established a wellness committee to develop the District's proposed local wellness policy, making such policy recommendations for review and adoption by the Board of Education. The District's wellness committee includes, but is not limited to, representatives from each of the following groups:
 

  1. Parents;

  2. Students;

  3. Physical Education teachers;

  4. School health professionals;

  5. The District's food service program;

  6. The School Board;

  7. School administrators; and

  8. Members of the public.

The District Wellness Committee will assess current activities, programs and policies available in the District; identify specific areas of need within the District; develop the policy; and provide mechanisms for implementation, evaluation, revision and updating of the policy. The Wellness Committee is established to represent the local community’s perspective in developing the wellness policy for the District.
